A sports or college physical is not complicated, but it does need to be done right and done by a qualified provider who knows what schools and athletic programs are actually looking for. Here is how our process works from start to finish.

Walk in or schedule same-day — no primary care referral needed

You do not need a referral or an existing relationship with our clinic to get a sports or college physical here. Walk in when it works for you, or call ahead to confirm availability. In most cases, we can see you the same day.

This matters a lot when tryouts sneak up, a college deadline is closer than you thought, or you simply do not want to wait three weeks for a primary care opening. We built our schedule around the reality that physicals often come with a deadline attached.

Get evaluated by experienced providers who know what schools and leagues require

Once you are in, you will be examined by an experienced provider working under the direct oversight of Dr. Thaer Joudeh, MD, Southside Medical & Laser Center’s board-certified internal medicine physician. For sports physicals, that means reviewing your medical history, checking cardiovascular function, evaluating musculoskeletal health, assessing vision and hearing, and screening for conditions that could put you at risk during athletic activity.

For college physicals, the exam covers what most institutions require for enrollment, general health assessment, immunization review, and documentation that meets your school’s specific requirements. If you need additional testing or updated vaccinations to complete your college health forms, we can handle that in the same visit or refer you to the right next step without sending you on a runaround.

The whole point of coming in is leaving with something you can actually hand to your coach, your school’s athletic office, or your college health center. Our providers complete and sign the required forms before you walk out.

What is a sports physical and why does your student need one?

A sports physical — formally called a Pre-Participation Physical Evaluation (PPE) — is the standard exam required by most middle schools, high schools, and recreational leagues before a student athlete is allowed to participate. South Carolina schools follow the NFHS guidelines for pre-participation exams, and the clearance form your child comes home with needs to be completed by a licensed medical provider.

The purpose is not just paperwork. A well-done sports physical screens for heart conditions, musculoskeletal issues, and other health factors that could put a young athlete at risk during intense physical activity. Conditions like undiagnosed hypertrophic cardiomyopathy, one of the leading causes of sudden cardiac death in young athletes, are exactly what this exam is designed to catch.

What does a sports physical include?

Our sports physicals cover the full Pre-Participation Physical Evaluation, including:

  • Medical and family history review — prior injuries, surgeries, hospitalizations, and family history of heart conditions
  • Cardiovascular screening — blood pressure, pulse, heart and lung sounds
  • Musculoskeletal assessment — joint flexibility, strength, and any history of prior injuries
  • Vision and hearing screening
  • Height, weight, and BMI
  • Review of any current medications or supplements
  • Clearance documentation completed and signed by our provider

If your student has a known condition, asthma, a prior concussion, a repaired joint, we note it and clear them appropriately, or explain exactly what additional evaluation is needed before they can be cleared.

When should you schedule a sports physical?

The best time is four to six weeks before the season starts or tryout date. That gives enough time to address anything that comes up during the exam without missing the first practice. Waiting until the week before means any follow-up could cost your student their spot.

That said, if you are running close to the deadline, come in anyway. We will do our best to get everything completed the same day.

College Physicals and Health Clearance Forms

What colleges typically require and why it matters

Most colleges and universities require an incoming student health form, sometimes called a college physical or pre-enrollment health clearance, before the student can complete registration, move into housing, or participate in athletics. These requirements vary by school but typically include a general physical examination, immunization records review, and documentation of any current health conditions.

Some schools require specific immunizations like meningococcal vaccine, varicella, MMR, or Tdap. If your student is missing any required vaccines, we can administer them in the same visit and provide the documentation your school needs.

How long does a sports or college physical take?

Most visits take between 20 and 40 minutes, depending on the complexity of the medical history and whether any additional documentation or vaccinations are needed. Same-day walk-in appointments are typically seen within a reasonable wait time.

Do I need to bring anything to the appointment?

For sports physicals, bring the form your school or league requires, most schools have a specific form that needs to be completed and signed. For college physicals, bring your school’s health forms and a record of your immunization history if you have one. If you do not have immunization records, we can work with what you have and advise on what may need to be updated.

Can a parent bring multiple children in the same visit?

Yes. We see siblings in the same visit. If you have two or three kids who all need sports physicals before the same deadline, come in together and we will work through each one.

What if my child gets flagged for something during the exam?

Being flagged does not automatically mean they cannot play. It means a specific finding needs follow-up before clearance can be issued, often something straightforward like an EKG to rule out a cardiac concern, or a referral to a specialist for a prior knee injury. We explain exactly what is needed and why, so you are not left guessing.

Does insurance cover sports and college physicals?
